You have too many bad sector in your hard disk. this is good decision to backup your data first. I think you need to Surface Test With Mini Partition Tool.
and if you want to repair your disk Health Try With HD Re generator but this take a long time. but not 100% Repair

I would not ask you to replace the drive right away.

First, run the chkdsk /r and then check its health with crystal disk

Hi i did that chkdsk /r but same changed. Sad maybe it failed.
yeah the hard drive is bad time to dump it
